Job summary

CVS Health in New Jersey seeks a Registered Nurse Case Manager to coordinate care for members with complex needs living at home. This field-based role supports safety, independence, and quality of life across Morris and surrounding counties.

You will conduct in-home assessments, evaluate medical, behavioral, and social needs, collaborate with providers and caregivers, develop personalized care plans, document in the EHR, and mentor new staff. Travel up to 75% is required.

Qualifications

• Associate's degree in Nursing is required; a BSN is preferred.
• At least 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings.
• Active, unrestricted RN license in New Jersey; NJ Choice Certification with an 80% exam score and required training modules.

Responsibilities

• Conduct in-home assessments for members enrolled in MLTSS programs.
• Evaluate medical, behavioral, and social needs to support appropriate care.
• Collaborate with providers, caregivers, and interdisciplinary teams to ensure coordinated, holistic care.
• Develop and manage individualized care plans that promote safety and independence.
• Participate in care team meetings and advocate for member needs.
• Document assessments and care activities accurately in the EHR.
• Support members through care transitions and connect them with community resources.
• Mentor new team members as you grow in the role.
