Job summary

UnitedHealth Group seeks an RN Clinical Care Coordinator to manage a panel of members with complex needs. This field-based role offers a home-based office, up to 75% local travel, and remote work when not in the field.

You will assess, plan, and coordinate medical, behavioral, and social resources to improve quality of care. Requirements include MA RN licensure, 2+ years of clinical experience, and strong MS Office skills.

Qualifications

• 2+ years of clinical experience
• Proficiency with MS Word, Excel, and Outlook
• Access to a quiet home workspace with PHI security
• Reliable internet connection and transportation within territory
• Valid US driver’s license

Responsibilities

• Engage members for needs assessments covering medical, behavioral, functional, cultural and socioeconomic needs
• Develop and implement person-centered care plans addressing chronic conditions and wellness
• Collaborate with internal care team, providers and community resources
• Educate and coach members for self-management and healthy lifestyle changes
• Coordinate discharge planning and care transitions following ER or SNF visits
• Advocate for members to ensure needs and choices are represented
