Qualifications

Responsibilities

• Coordinate care for medically complex members and perform assessments.
• Apply evidence-based practices to care coordination activities.
• Deliver member-centered self-management support and patient education.
• Ensure appropriate utilization and consistent application of health benefits.
• Conduct telephonic and face-to-face assessments per policy.
• Advocate for members and resolve barriers to care.
• Coordinate referrals to community resources with members and providers.
• Develop and evaluate individual care plans with the Care Coordination Team.
• Monitor hospital encounters and provide education or interventions as needed.
