Job summary

UnitedHealthcare is seeking an RN Clinical Care Coordinator to serve as the primary care manager for a panel of members with complex medical and behavioral needs. Expect about 80% field time visiting members in their homes or long-term care facilities in Mercer and surrounding counties, NJ.

Qualifications

• Current, unrestricted RN license in New Jersey.
• 2+ years of clinical experience.
• Ability to travel up to 80% within assigned territory to meet with members/providers.

Responsibilities

• Engage members face-to-face and/or telephonically for comprehensive needs assessments.
• Develop and implement person-centered care plans for chronic conditions, wellness, and social determinants.
• Coordinate with care team, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs.
• Support discharge planning and care transitions after ER/SNF events.
• Advocate for members to ensure needs and choices are represented by the care team.
