Job summary
21st Century Home Health Services in Hayward, CA is seeking a Physical Therapist Case Manager, Level 2, to evaluate and address the needs of individuals in their homes, helping them live safely and productively.
You will supervise 2 PTAs, develop care plans, coordinate with physicians, and adapt therapies to restore function and safety in the home setting.

Qualifications
- CA PT license in good standing and unrestricted.
- Experience supervising PTAs in home health environment preferred.
- Current BLS/CPR certification.
- Valid CA driver’s license.
- Knowledge of local resources and home health regulations.
- Experience with high acuity patients and interdisciplinary teams preferred.
- Self-directed, empathetic and strong communication skills.
Responsibilities
- Evaluate patients in their homes and develop care plans with PTA collaboration.
- Address mobility and independence goals to maximize functional recovery.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure quality care.
- Evaluate and report patient progress to the physician while supervising PTAs.
