Job summary

ForzaCare, a field-based care coordination provider, is seeking a Field Medical Case Manager to guide injured workers through recovery and safe return-to-work processes. You will serve as the central point of contact among workers, medical providers, employers, and insurers to ensure timely, transparent, and effective care management.

Ideal candidates are licensed RNs or CRCs, with experience in workers' compensation and a strong focus on collaboration, communication, and compliance.

Qualifications

• Active RN or CRC license required with associated college degree.
• Valid driver's license, reliable transportation, and ability to travel to appointments.
• Prior workers' compensation case management experience preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, including visits to workplaces as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
