Job summary

ForzaCare is seeking a Field Medical Case Manager who will coordinate care for injured workers, acting as the central point between medical providers, employers, and insurers to ensure timely, transparent care management.

The ideal candidate is a licensed RN or CRC with workers' compensation experience, travel readiness, and a commitment to high-quality, compliant service. CCM/CIRS credentials are a plus.

Qualifications

• Active RN or Certified Rehabilitation Counselor license is required.
• Bachelor’s degree is required.
• Must comply with state licensure requirements.
• Prior workers' compensation case management experience is strongly preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Coordinate care among medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at employers and with injured workers.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure all case management work meets customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal team members.
