Job summary

ForzaCare is seeking a Field Medical Case Manager to guide injured workers through recovery and return-to-work processes. You will act as the central coordinator among medical providers, employers, and insurers to ensure timely, transparent care management.

Ideal candidates hold an active RN or CRC license and may have CCM or CIRS credentials. Travel to appointments is required, and experience in workers' compensation is highly valued.

Qualifications

• Active RN or Certified Rehabilitation Counselor license with associated college degree.
• CCM, CIRS, or other case management credentials are preferred.
• Must comply with state licensure and certification requirements.
• Prior experience in workers' compensation case management is strongly preferred.
• Valid driver's license, reliable transportation, and ability to travel to appointments.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with injured workers, potentially visiting employers and workers at their place of employment.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets or exceeds customer and compliance requirements.
• Build and maintain strong relationships with clients, providers, and internal teams.
