Job summary

ForzaCare in California seeks a Field Medical Case Manager to coordinate care for injured workers, connect them with medical providers, employers, and insurers, and ensure timely, transparent case management. This role is ideal for licensed nurses or certified rehabilitation counselors.

While prior workers' compensation experience is strongly preferred, candidates from related clinical fields with strong coordination skills will be considered.

Qualifications

• Active RN or CRC license with college degree is required.
• CCM or CIRS credentials preferred.
• Must comply with state licensure requirements.
• Prior workers' compensation case management experience strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurers, and injured workers.
• Attend appointments, including at workplaces as needed.
• Develop and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and progress reports to clients and stakeholders.
• Ensure case management meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and team members.
