Job summary

ForzaCare in the United States is seeking a Field Medical Case Manager to help injured workers navigate recovery and return to work. You will serve as the central coordinator linking workers, providers, employers, and insurers to ensure timely, clear care management.

Ideal candidates are licensed RNs or CRCs; prior workers' compensation case management is strongly preferred. You will travel to appointments, support goal setting, and document progress for clients and stakeholders.

Qualifications

• Active RN or CRC licensure required; degree on file.
• CCM/CIRS or similar case-management credentials preferred.
• Must comply with state licensure requirements and travel to appointments.
• Prior experience in workers' compensation case management strongly preferred.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments with the injured workers and may visit employers' and workers' workplaces.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide consistent communication and detailed progress reports to clients and stakeholders.
• Ensure all case management work meets safety, ethical, and professional standards; build relationships with clients and providers.
