ForzaCare is seeking a Field Medical Case Manager to coordinate care for injured workers, connecting medical providers, employers, and insurance carriers to support timely recovery and return-to-work. The role suits licensed nurses or certified rehabilitation counselors, with strong preference for workers' compensation experience.

Travel to appointments and sites is expected; maintain compliance with safety and professional standards while delivering high-quality care.

Qualifications

Active RN license with relevant college degree.
CRC, CCM, or other case management credentials preferred.
Prior workers' compensation case management experience preferred.
Valid driver’s license and reliable transportation required.
Willingness to travel to appointments and sites as needed.

Responsibilities

Coordinate care among medical providers, employers, insurers, and injured workers.
Attend appointments at employer or worker locations as needed.
Develop and monitor individualized recovery goals and return-to-work plans.
Provide regular, detailed progress reports to clients and stakeholders.
Ensure all case management work meets customer and compliance requirements.
Build and maintain strong relationships with clients, providers, and team members.
