Job summary

ForzaCare is seeking a Field Medical Case Manager to help injured workers navigate recovery and return-to-work. You will coordinate between medical providers, employers, and insurers to ensure timely, transparent care management.

This role is ideal for licensed nurses or certified rehabilitation counselors. Prior workers' compensation experience is strongly preferred and travel to appointments is required.

Qualifications

• Active RN license or CRC certification is required.
• CCM, CIRS, or other case management credentials are preferred.
• Must comply with state licensure and certification requirements.
• Prior workers' compensation case management experience is strongly preferred.

Responsibilities

• Coordinate care between medical providers, employers, insurance carriers, and injured workers.
• Attend appointments at workplaces or other sites as needed.
• Develop, document, and monitor individualized recovery goals and return-to-work plans.
• Provide regular, detailed progress reports to clients and stakeholders.
• Ensure case management work meets customer and compliance requirements.
• Build and maintain relationships with clients, providers, and internal teams.
