Job summary

Humana is seeking a Care Coordinator to support Illinois Medicaid Members within the Humana Gold Plus Integrated program. You will assess needs, develop personalized care plans, and connect members with resources to improve health and well-being.

Travel 75–90% to visit members in homes and care facilities; remote/field role with flexible scheduling. Required qualifications include an Illinois-resident, 1+ year in health care or case management, and experience with Medicare/Medicaid or long-term

Qualifications

• Must reside in Illinois and have 1+ year in health care and/or case management.
• Experience with Medicare & Medicaid recipients and/or long-term care.
• Proficiency with MS Word, Excel, Outlook and EMR systems.
• Licenses or degrees listed in qualifications must be current.

Responsibilities

• Visit Medicaid members in homes, facilities, or care settings; travel 75–90%.
• Assess care needs to ensure services in least restrictive settings.
• Identify and remove barriers hindering care delivery.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care plan against goals.
• Track progress toward outcomes via assessment and engage in care planning.
• Assess enrollees’ needs via telephonic and face-to-face encounters.
• Assist with remediation of gaps in care and access.
• Assist with filing grievances and appeals.
• Provide information on MCO requirements, services, benefits, resources.
• Refer enrollees to community resources and guide to appropriate care.
• Coordinate with payers, providers, and interdisciplinary teams as needed.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in Illinois with 1 year of experience

Licensed Professional Counselor (LPC)

Licensed Social Worker (LSW)
