Job summary

Elevance Health is seeking a LTSS Service Coordinator Clinician to operate in the field, traveling to Johnson County, IN and Shelby County, IN to coordinate care under RN supervision. This role supports identification, evaluation, and management of members’ health and social needs across physical, behavioral health, and long-term services.

Minimum qualifications include a licensure (LPN/LVN, LSW, LCSW, LMSW or RN-credential as allowed by state law) and at least 2 years’ experience with chronic

Qualifications

• LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law.
• Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) required.
• May require state-specific certification based on state law and/or contract.

Responsibilities

• Assists the responsible RN in identifying members for high risk complications.
• Obtains clinical data as directed by the RN.
• Assists the RN in identifying members that would benefit from an alternative level of care or other waiver programs.
• Provides information collected to the RN to verify, interpret, and develop the member's care plan.
• Coordinates care for members with chronic illnesses and disabilities to ensure cost-effective utilization of health benefits.
• Decision making will be based on member needs with understanding of disease processes; nursing judgment not required.
