Job summary

Elevance Health in Indiana seeks a LTSS Service Coordinator Clinician who will work under an RN's supervision, handling assessments, care planning and coordination across physical health, behavioral health, social services and long-term supports.

Candidates should reside in Johnson or Shelby counties, Indiana, and be willing to travel to client sites; hybrid work policy applies, with some weekly onsite requirements.

Qualifications

• LPN/LVN or social work license per state law.
• Minimum 2 years with chronic illnesses in care coordination.
• Current unrestricted license in applicable state(s).

Responsibilities

• Assist the RN in identifying high-risk members.
• Obtain clinical data as directed.
• Assist in identifying members for alternative levels of care or waiver programs.
• Coordinate care with the RN and health team to ensure appropriate utilization of benefits.
• Provide telephonic or face-to-face assessments as needed.
