Job summary

Elevance Health is seeking a Field-based LTSS Service Coordinator to support a defined caseload in specialized programs. You will conduct assessments, document person-centered plans, and collaborate with a multidisciplinary team to align health, behavioral health and LTSS needs with state and contractual requirements.

The role involves travel to client sites within Indiana, with some office attendance for meetings and training as needed.

Qualifications

• BA/BS degree required.
• Minimum 2 years of experience with a social work agency.
• Must meet state law and contract requirements.
• Travel to worksite and other locations as necessary.

Responsibilities

• Conduct face-to-face program assessments for waiver identification.
• Coordinate member cases with clinical teams to provide care coordination.
• Manage non-clinical needs for members with chronic illnesses.
• Document member goals with their care team and caregivers.
• Identify members who may need alternative service levels or other waivers.
• Support training of associates and assist process improvements.
• Submit utilization/authorization requests with care plan alignment.
• Report critical incidents to state and county agencies as required.
• Assist with appeals, hearings, and audits as needed.
