Job summary

UnitedHealth Group is seeking a Case Manager in Colorado to provide long-term services and supports (LTSS) case management. You will conduct intake, assessments, develop and monitor care plans, and document activities in the prescribed system.

Travel within Lake, Chaffee, Fremont or Custer Counties is required, with up to 75% travel and possible remote work in certain counties. The role emphasizes confidentiality and compliance with CMA requirements.

Qualifications

• MS Office proficiency (Word, Excel, Outlook).
• Bachelor’s degree in human behavioral sciences or related field; combination of education and experience may be accepted.
• Access to reliable transportation and ability to travel up to 75% within assigned territory.

Responsibilities

• Lead intake/screening/referral, assessment and development of care plans; ongoing case management and documentation.
• Maintain confidentiality in accordance with policies and the Employee Handbook; ensure secure handling of patient information.
