Job summary

Humana offers a field-based Transition/Custodial Prevention Specialist role focused on helping members navigate long-term services and supports. You will assess goals, coordinate services, and empower independence through person-centered planning.

Responsibilities include coordinating care with facility staff, identifying LTSS providers, and advocating for access to behavioral health services. This role requires travel within Illinois and surrounding regions.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Two years of experience with Medicare/Medicaid recipients or HCBS.
• One year or more in custodial and transitional care.
• Experience with electronic information applications/EMR.

Responsibilities

• Meet members in the field at home, NF, or location chosen by the member to assess goals and needs.
• Help develop plans of care and connect members with LTSS providers and community resources.
• Coordinate services and advocate for member care within an interdisciplinary team.
• Educate members to maintain Medicaid eligibility and navigate LTSS.
• Assist with LTSS entry and use of state portals.
• Travel 75-90% to meet with members and families.
