Elevance Health is seeking a LTSS Service Coordinator Clinician to support field-based care coordination across Fulton, Wabash, and Miami counties. The role involves working under the supervision of an RN to assess, identify needs, and coordinate health and long-term services for members.

The position emphasizes travel to member sites, collaboration with the health care team, and adherence to state law and contract requirements.

Qualifications

• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) is required.
• Minimum of 2 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Experience in coordinating care and applying clinical guidelines in LTSS.
• Willingness to travel to designated counties and work across multiple sites.

Responsibilities

• Support the RN in identifying members for high-risk conditions.
• Collect clinical data as directed by the RN.
• Assist in identifying members who may need alternative levels of care or waiver programs.
• Develop and monitor care plans in collaboration with the health care team.
