Job summary

Elevance Health seeks a LTSS Service Coordinator to support a defined caseload in Maury County, TN. This field-based role requires traveling to member sites and coordinating with the care team to develop person-centered plans.

You will conduct assessments, document goals, and collaborate with medical directors and the interdisciplinary team to align PH/BH/LTSS needs within state and contract requirements. Occasional office attendance may be required.

Qualifications

• BA/BS degree as required or equivalent experience.
• Minimum 2 years with a social work agency or equivalent background.
• State-specific educational requirements may apply.
• Strong communication and organizational skills.

Responsibilities

• Face-to-face program assessments using predefined questions to identify needs and coordinates waiver or BH/PH supports.
• Identify members with potential clinical health care needs and coordinate with the clinical team as the single point of contact.
• Manage non-clinical needs of members to ensure efficient utilization of LTSS and related services.
• Document short- and long-term goals in collaboration with the member's care team for PCSP and backup plans.
• Identify members who would benefit from alternative levels of service or other waiver programs.
• May mentor or precept new staff and participate in process improvement initiatives.
• Submit utilization/authorization requests supporting the care plan to utilization management.
• Report critical incidents to appropriate parties and participate in state audits and appeals processes.
