Job summary

Elevance Health is seeking an LTSS Service Coordinator who will serve Knox or Davidson and surrounding counties in TN. This field-based role requires travel to client sites with occasional office attendance for meetings and training.

The coordinator documents and updates the Person Centered Plan, interfaces with Medical Directors, and participates in interdisciplinary care rounds to support integrated care.

Qualifications

• Requires BA/BS degree and a minimum of 2 years of experience working with a social work agency.
• Specific education, years, and type of experience may be required based upon state law and contract requirements.
• Travel to worksite and other locations as necessary.

Responsibilities

• Perform face‑to‑face program assessments for identification and coordination of waiver needs.
• Identify members with potential clinical health care needs and coordinate with clinical oversight and care teams.
• Manage non‑clinical needs of members to ensure cost‑effective LTSS utilization.
• Document member goals in collaboration with the care team and supports.
• Submit utilization/authorization requests with documentation supporting the care plan.
• Report critical incidents to state and county agencies as required.
