Job summary

Elevance Health is seeking a LTSS Svc Coordinator-Clinician to operate primarily in the field, traveling to client sites as required and attending meetings or training at times. This role supports RN-led care coordination for members with chronic conditions, ensuring effective care plans and appropriate utilization of benefits.

The position emphasizes collaboration with health care teams, data gathering per RN directions, and adherence to the company’s hybrid/work location policy.

Qualifications

• Requires LPN/LVN, LSW, LCSW or LMSW license per state law with Nursing Diploma or AS in Nursing.
• Minimum 2 years of experience with chronic illnesses, co-morbidities or disabilities in a service coordination role.
• Equivalent education/experience options allowed.

Responsibilities

• Assists responsible RN in identifying members for high risk complications.
• Obtains clinical data as directed by the responsible RN.
• Assists the responsible RN in identifying members that would benefit from an alternative level of care or other waiver programs.
• Provides all information collected to the responsible RN, who verifies and interprets the information, conducts additional assessments, and develops, monitors, evaluates and revises the member's care plan.
