Job summary

6 Degrees Search is seeking an experienced Hospice Sales Liaison to cover Rossville and surrounding areas. The role focuses on developing and maintaining strong referral-source relationships with physicians, hospitals, discharge planners, case managers, SNFs and senior communities to grow census.

You will work field-based, balancing outbound visits with coordination with clinical and operations teams to ensure smooth transitions into care.

Qualifications

Hospice and/or Home Health Sales experience required.
Minimum 3 years of healthcare sales, marketing, business development or referral-development experience strongly preferred.
Established local contacts/referral sources strongly preferred.
Local market knowledge and proven, verifiable performance are must-haves.
Ability to establish and expand relationships with diverse referral sources.
Familiarity with CRM/contact management software preferred.
Strong problem-solving skills and ability to work independently in the field.
Reliable transportation, valid driver's license and current auto insurance required.

Responsibilities

Develop, expand and maintain strong referral-source relationships across the assigned market.
Represent hospice services professionally with referral partners, patients, families and community partners.
Identify new referral opportunities and strengthen existing accounts through consistent outside sales activity.
Work closely with internal clinical and operations teams to support appropriate referrals and smooth transitions into care.
