The role focuses on developing and maintaining strong referral-source relationships with physicians, hospitals, discharge planners and case managers to grow census and ensure smooth transitions into care. The ideal candidate has hospice/home health sales experience, local market knowledge, a proven performance record, and the ability to manage diverse referral partners.

Qualifications

• Hospice and/or Home Health sales experience required.
• Minimum 3 years of healthcare sales, marketing, business development or referral development experience preferred.
• Established local contacts/referral sources preferred.
• Local market knowledge, stability and verifiable performance are must-haves.
• Ability to establish and expand relationships with diverse referral sources.
• Familiarity with CRM/contact management software preferred.

Responsibilities

• Develop, expand and maintain strong referral-source relationships across the assigned market.
• Represent hospice services professionally with referral partners, patients, families and community partners.
• Identify new referral opportunities and strengthen existing accounts through consistent outside sales activity.
• Work with clinical and operations teams to support referrals and smooth transitions into care.
