Job summary

Kona Medical Consulting and The Home Health Team seeks a Home Health Field Registered Nurse (RN) to provide skilled nursing care in patients' homes across the Dallas–Fort Worth Metroplex. The role emphasizes reliable clinical decision-making, compassionate care, and autonomy in daily workflow.

Ideal candidates are organized, customer-focused, and committed to delivering high-quality care with support from leadership and a strong interdisciplinary team.

Qualifications

• Texas RN license required.
• 1+ year nursing experience (home health preferred).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
