Job summary

HealthFlex Home Health Services is seeking a Field Clinical Supervisor to blend direct field nursing with office-based case management to support patient outcomes and staff development. You will conduct SOC evaluations, lead field supervision, and oversee utilization review, while maintaining hands-on patient care in the field.

You will mentor clinical staff, participate in interdisciplinary case conferences, and ensure quality and regulatory compliance.

Qualifications

• Current, unrestricted California RN license to practice as an RN.
• Strong written and verbal communication skills with the ability to train staff.

Responsibilities

• Perform Start of Care nursing evaluations and SOC assessments.
• Lead field supervisory visits for RNs, LVNs, HHAs, and other staff.
• Provide hands-on training and onboarding for new hires.
