Job summary

HealthFlex Home Health & Hospice is seeking an experienced Physical Therapist to become Field Clinical Supervisor. This role blends hands-on patient care with office-based case management to support patient outcomes, regulatory compliance, and staff development.

The Field Clinical Supervisor will perform SOC evaluations, supervise field staff, and provide training while maintaining caseload coverage and ensuring quality care across the organization.

Qualifications

• Current, unrestricted California Physical Therapist license in good standing.
• Strong written and verbal communication skills; ability to train and mentor clinical staff effectively.
• Proficiency with WellSky (or willingness to train) and Paylocity or similar HRIS.

Responsibilities

• Perform Start of Care (SOC) evaluations and supervise field PTAs and PTs.
• Conduct field supervisory visits and support annual performance reviews.
• Deliver hands-on patient care when needed to cover caseloads and ensure quality outcomes.
• Oversee scheduling, utilization review, and case management to ensure regulatory compliance.
• Lead staff education to reduce rehospitalization and improve care coordination.
