Job summary

Qualifications

• Bachelor’s degree in human services or a related field is required with at least 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities.
• Travel up to 40% of time to visit members; valid state driver’s license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services per MCP.
• Travel to members location for assessments and quarterly visits; maintain monthly contact by phone.
• Arrange support services including social integration, community resources, housing, and other non-medical needs.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
