Enlyte is seeking a Field Case Manager in a full-time, hybrid role based in Milwaukee, WI. You will coordinate care with treating physicians, employers, and patients to implement effective treatment plans.

The role emphasizes independent case management, collaboration with healthcare providers, and ensuring timely return-to-work strategies, while maintaining quality care and cost efficiency.

Qualifications

• RN license required and in good standing.
• Associates degree or Bachelor’s degree in Nursing or related field.
• 2+ years clinical practice preferred; workers’ compensation experience preferred.
• Proficient in Excel, Word and Outlook; strong written communication.
• Spanish fluency is a plus; URAC-certified CM preferred (CCM, CDMS, CRC, CRRN, COHN, COHN-S, RN-BC, ACM, CMAC, CMC).

Responsibilities

• Demonstrate knowledge and competency in case management standards of practice.
• Advise on treatment plans using knowledge of injuries, medications, and job duties.
• Interview injured workers to assess recovery and occupational factors.
• Collaborate with physicians and employers to tailor treatment plans.
• Modify job duties with employers and physicians to enable early return to work.
• Manage workload by prioritizing cases and efficient time management.
• Attend appointments when needed and prepare status updates for customers.
