Job summary

Moms In Motion is seeking a Service Facilitator to support families in Virginia, traveling locally to client homes to help navigate Medicaid Waiver programs, ensuring safe, healthy, and independent living. No degree is required; heart and experience matter, with emphasis on compassionate, organized, community-focused service.

You will visit clients, guide families through waiver options, write care plans, troubleshoot authorizations, and build trusted relationships.

Qualifications

• 2+ years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter.
• Proficient with email, documents, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle.
• Great written and spoken communication.
• Able to pass a background check and provide 2 references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients at their homes locally.
• Guide families through Virginia waiver programs.
• Write care plans and assessments that make a difference.
• Troubleshoot authorizations, timesheets and related tasks.
• Build lasting relationships with families based on respect and trust.
• Work remotely from a laptop/tablet.
