Job summary

Based in Virginia, Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will travel locally to client homes and help families navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). No degree required and commitment to compassionate service is essential.

You’ll write plans of care, assist with authorizations and timesheets, and build trusting relationships with families, all while working from a laptop/tablet. Earnest, organized helpers welcomed.

Qualifications

• Minimum 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter.
• Proficient with email, documents, spreadsheets, portal navigation and video calls.
• Valid driver’s license and access to a reliable vehicle.
• Excellent written and verbal communication.
• Able to pass a background check and provide 2 references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes locally.
• Guide families through waiver programs.
• Write plans of care and assessments that make a difference.
• Troubleshoot service authorizations, timesheets and related tasks.
• Build lasting relationships with families based on respect, patience and trust.
• Work remotely from a laptop/tablet.
