Job summary

Moms In Motion in Virginia is seeking Service Facilitators to visit clients in their homes and guide families through Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). The role emphasizes compassionate, organized, and community-focused service, with local travel and a strong emphasis on building trusted relationships.

No degree is required, but at least 2 years of relevant experience is preferred. A valid driver's license, reliable vehicle, and excellent communication are essential.

Qualifications

• Minimum 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more.
• Must be tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle (no client transport).
• Excellent written and spoken communication.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes on a local schedule.
• Serve as the go-to guide for families navigating waiver programs.
• Prepare plans of care and assessments that make a difference.
• Troubleshoot issues like service authorizations and timesheets.
• Build lasting relationships with families based on respect and trust.
• Work remotely from a laptop/tablet as needed.
