Job summary

Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will ensure families in Virginia receive compassionate, well-coordinated support. You’ll visit clients at home, guide them through waiver programs, and develop plans of care that improve safety, health, and independence.

Role offers remote work from a laptop or tablet with local travel, and requires living in Virginia with the ability to travel to client homes daily.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required.
• Proficient with email, documents, spreadsheets, portal navigation and video calls.
• A valid driver’s license and reliable vehicle.
• Able to pass a background check and provide 2 professional references.
• Bonus points if you have Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road (locally) to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
