Job summary

Moms In Motion in Virginia is seeking a Service Facilitator to connect with families, helping them navigate Medicaid Waiver programs and ensuring safe, healthy, independent living. You’ll visit client homes locally and work from a laptop.

No degree is required, but you’ll need at least 2 years of experience supporting individuals with disabilities or the elderly, plus a valid driver's license and reliable vehicle. Excellent communication is essential.

Qualifications

• Excellent communication skills, both written and spoken.
• Must have a valid driver's license and reliable vehicle.
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• 2+ years of experience supporting individuals with disabilities or the elderly.
• No degree required – heart and experience matter more than formal credentials.

Responsibilities

• Visit clients locally in their homes.
• Be a go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that make a difference.
• Troubleshoot issues like service authorizations and timesheets.
