Job summary

Moms In Motion seeks a Service Facilitator (SF) Case Manager in Virginia to visit families in their homes, helping them navigate Medicaid Waiver programs and ensuring safe, healthy, independent living.

No degree is required; at least 2 years of experience with disabilities or the elderly is preferred. You will travel locally, write plans of care, troubleshoot authorizations and timesheets, and build trusting relationships while working from a laptop.

Qualifications

• Experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Valid driver’s license and reliable vehicle.
• Pass background check and provide 2 professional references.

Responsibilities

• Visit clients in their homes as part of daily local travel.
• Guide families through Medicaid Waiver programs.
• Write care plans and assessments that drive outcomes.
• Troubleshoot authorizations, timesheets, and related tasks.
• Build lasting relationships with families based on respect and trust.
