Moms In Motion is seeking a Service Facilitator (SF) Case Manager to visit clients in their homes across Virginia and guide families through Medicaid Waiver programs. You will assess needs, write care plans, and ensure services are delivered with empathy and efficiency.

The role requires at least 2 years of experience with disabilities or the elderly; no degree is required. A valid driver’s license, reliable vehicle, and ability to pass a background check are essential.

Formación

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience prioritized.
• Proficient with email, documents, spreadsheets, portal navigation, and video calls.
• Valid driver’s license and reliable vehicle (no client transport).
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person-Centered Thinking/Planning training.

Responsabilidades

• Visit clients in their homes locally in VA.
• Guide families through Virginia Medicaid Waiver programs.
• Write plans of care and assessments that actually make a difference.
• Troubleshoot service authorizations, timesheets, and more.
• Build lasting relationships with families based on respect, patience, and trust.
• Work remotely from your laptop/tablet.
