Ideal candidates hold a Bachelor’s degree or non-licensed master's in behavioral health or human services, with 2+ years in behavioral health or related fields and proficiency in MS Office; QuickBase is a plus.

Qualificações

• 2 years' experience in behavioral health, social services, or appropriate related field.
• West Virginia residency in one of the specified counties.
• Must possess reliable transportation and travel 50% or more of the time.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint, Teams).

Responsabilidades

• Conduct comprehensive evaluation of referred member's needs/eligibility and recommend an approach to case resolution.
• Identify high risk factors and refer to clinical case management or crisis intervention as appropriate.
• Coordinate and monitor care plan activities and progress.
