Job summary

CVS Health is seeking a full-time field-based Care Manager to support Aetna Better Health of West Virginia. This role travels to member homes, facilities, and community locations to evaluate needs and coordinate care plans.

The position requires WV residency, county coverage, and the ability to travel 50% or more. A bachelor's or master’s degree in behavioral health or human services is preferred, with strong MS Office skills and clinical collaboration.

Qualifications

• 2 years' experience in behavioral health, social services, or related field.
• WV resident in Barbour, Brooke, Doddridge, Hancock, Harrison, Lewis, Marion, Marshall, Monongalia, Ohio, Pleasants, Ritchie, Taylor, Tyler, Upshur, Wetzel counties.
• Must have reliable transportation and be willing to travel 50%+ within WV region.
• 2+ years PC experience and MS Office proficiency (Outlook, Word, Excel, SharePoint, Teams).

Responsibilities

• Evaluate referred members' needs/eligibility and plan care resolution.
• Identify high risk factors and coordinate referrals to clinical care.
• Coordinate and monitor care plan progress.
• Ensure compliance with regulatory and internal guidelines.

Education

Bachelor's degree in behavioral health or human services
