Job summary

Qualifications

• 2 years’ experience in behavioral health or related field.
• West Virginia residency in one of the specified counties and travel 50%+.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint, Teams).

Responsibilities

• Evaluate referred members’ needs and eligibility to tailor care plans.
• Identify high-risk factors and coordinate referrals to clinical care.
• Coordinate care plan activities and monitor progress.
