Qualifications

• Proficient with MS Office (Outlook, Word, Excel).

Responsibilities

• Evaluate referred members' needs/eligibility and plan care.
• Identify high-risk factors and coordinate care plans with appropriate referrals.
• Coordinate and monitor care plan progress.
• Collaborate with case managers, supervisors, and Medical Directors to improve outcomes.
• Coach members to participate in decisions and healthy lifestyle changes.
