Job summary

Department of Economic Security is seeking a Case Manager 2 - Targeted Unit in Prescott Valley, AZ. You will drive to member homes and schools, develop Person-Centered Service Plans, and coordinate with community agencies to provide essential supports.

The role requires a relevant degree or RN/license, 1–3 years of case management experience, strong documentation, and the ability to work in a fast-paced team. Desirable drive and strict compliance with regulations are expected.

Qualifications

• Arizona-licensed Registered Nurse in good standing or Bachelor in Social Work/Psychology/Special Education/Counseling with 1 year of relevant case management experience.
• Bachelor’s in Human Services or related field with 1 year of case management experience OR 3 years with disabilities.
• If driving essential, valid driver’s license and vehicle insurance; fingerprints and background checks required.
• Must pass E-Verify and related state background checks; ability to work with diverse populations.

Responsibilities

• Drive a vehicle to conduct onsite visits to members' homes, schools, and program sites.
• Develop, review, monitor and evaluate PCSP; refer to community services as needed.
• Facilitate meetings to assist member/family choices; complete referrals for services.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and maintain case records; prepare reports and complete special projects.
