The Department of Economic Security in Arizona is hiring a Case Manager 2 - Targeted Unit within the Division of Developmental Disabilities Specialty District in Apache Junction. Qualifications

• Arizona RN license or Bachelor’s in Social Work, Psychology, Special Education or Counseling with 1+ year case management experience; or 2+ years case management for elderly/disabled/SMI.
• Bachelor’s in Human Services or related field with 1+ year case management experience; or 3+ years experience with ID/DD populations.

Responsibilities

• Drive to member homes, schools or program sites to develop relationships and discuss service needs.
• Develop, review, monitor PCSPs and document progress and referrals.
• Facilitate meetings and program visits to support member/family choices.
• Collaborate with community agencies to provide assistance during crises.
• Write progress notes and maintain case records; prepare reports.
