Job summary

Arizona Department of Economic Security (DES) seeks a Case Manager 2 - Targeted Unit in Globe, AZ to support individuals with developmental disabilities. The role focuses on developing Person-Centered Service Plans, coordinating services, and collaborating with families and community partners to ensure timely, quality supports.

The selected candidate will drive to member homes and program sites, conduct assessments, document progress, and maintain confidential records in a team-oriented setting.

Qualifications

• Knowledge of HIPAA and health/behavioral health terminology.
• Experience developing Person-Centered Service Plans (PCSP).
• Experience in case management and coordinating services.
• Strong communication and interviewing skills.

Responsibilities

• Drive to member homes, schools, and program sites to discuss service needs.
• Develop, review, and monitor PCSP with timely documentation.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and maintain case records.
