Job summary

Qualifications

• Must have a valid Level One fingerprint clearance card.

Responsibilities

• Drive to conduct onsite visits to members’ homes and schools.
• Develop and monitor Person-Centered Service Plans (PCSP).
• Collaborate with community agencies to provide support.
