The Department of Economic Security (DES) in Arizona is seeking a Case Manager 3 to support the Division of Developmental Disabilities in Casa Grande. You will coordinate supports, develop Individual Support Plans, and collaborate with families, service providers and multidisciplinary teams to promote independence and well-being.

Applicants must meet pre-employment requirements including relevant licensure or degree and background checks.

Qualifications

• Experience with case management for elderly or disabled individuals.
• Ability to maintain confidentiality and work with diverse teams.
• Strong written and verbal communication skills.

Responsibilities

• Drive to on-site visits and develop relationships with members and families.
• Convene and facilitate plan meetings to determine services.
• Prepare reports and maintain case records; coordinate with providers.
• Lead team during supervisor absence and mentor coordinators.
