Job summary

The Department of Economic Security (DES) in Arizona seeks a Case Manager 3 – Targeted Unit in the Specialty District, Flagstaff. This role supports individuals with developmental disabilities and their families by developing Person-Centered Service Plans and coordinating services in the community.

Qualified candidates will have an RN license or a Bachelor's in Social Work/ Psychology/ Special Education/ Counseling and at least one year of case management experience, with driving required and

Responsibilities

• Drive a vehicle to conduct onsite visits to members' homes, schools or program sites
• Develop, review and monitor Person-Centered Service Plans and document progress
• Facilitate meetings and referrals for assessed/covered services
• Collaborate with community agencies to provide assistance to members and families in crisis
• Maintain case records and prepare reports and special projects, attend meetings and trainings
