Job summary

The Arizona Department of Economic Security is hiring a Case Manager 2 to provide high‑quality, member‑driven services for individuals with developmental disabilities in the Phoenix Metro area. This role focuses on supporting members and families in exercising independent decision‑making and engaging with community programs.

The candidate will conduct onsite visits, develop and monitor service plans, and collaborate with community agencies. Applicants must have a relevant degree and experience in case management for individuals with special needs.

Qualifications

• At least one year experience providing case management services to elderly or persons with disabilities.
• Experience working with individuals with intellectual or developmental disabilities.
• Valid Level One fingerprint clearance card.

Responsibilities

• Conduct onsite visits to members’ homes and schools.
• Develop, monitor, and evaluate Person‑Centered Service Plans.
• Collaborate with community agencies to provide assistance.

Skills

Developing, evaluating, and maintaining member service plans

Interviewing, counseling, and interacting with members and family members

Problem identification and resolution

Education

Bachelor Degree in Social Work, Psychology, Special Education or Counseling

Arizona licensed Registered Nurse in good standing

Bachelor’s Degree in Human Service, Behavioral Science or related field
