Job summary

Moms In Motion in Virginia is seeking a Service Facilitator to visit clients in their homes, helping families navigate Medicaid Waiver programs and develop plans of care that truly improve safety, health, and independence.

Position allows remote work from a laptop/tablet with daily local travel; you must reside in VA and have reliable transportation. The role emphasizes compassionate, organized, community-focused service and ongoing relationship-building.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience valued.
• Proficient with email, documents, spreadsheets, portal navigation and video calls.
• Valid driver's license and reliable vehicle (no client transport).
• Strong written and spoken communication.
• Able to pass a background check and provide 2 professional references.
• Bonus points if you have Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road (locally) to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
