Moms In Motion - A Virginia Service Facilitation Provider seeks Service Facilitator (SF) Case Manager to help families navigate Virginia’s Medicaid Waiver programs and ensure clients live safe, healthy, and independent lives. This role requires travel to client homes and remote work from a laptop or tablet.

Must live in VA with at least 2 years of relevant experience; degree is not required. Competitive benefits and comprehensive training are provided.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; emphasis on heart and practical experience.
• Proficient with email, word processing, spreadsheets, and video calls.

Responsibilities

• Visit clients in their homes for assessments and support.
• Guide families through Virginia Medicaid waiver programs.
• Write plans of care and assessments.
• Troubleshoot service authorizations and timesheets.
• Build lasting relationships with families based on respect and trust.
• Work remotely from a laptop/tablet.
