Job summary

Moms In Motion is seeking a Service Facilitator to visit clients in their homes across Virginia, supporting families navigating Medicaid Waiver programs and ensuring safe, healthy, independent living. You’ll document plans of care, address authorizations, and build trusted community relationships.

Candidates should have strong communication, organizational skills, a valid driver’s license, and the ability to work remotely from a laptop/tablet. Local travel is required.

Qualifications

• 2+ years of experience supporting individuals with disabilities or the elderly.
• No degree required; experience > formal education.
• Proficient with email, documents, spreadsheets, online portals, and video calls.

Responsibilities

• Hit the road to visit clients in their homes (local travel).
• Guide families through Virginia’s Medicaid Waiver programs.
• Write plans of care and assessments.
• Troubleshoot authorizations, timesheets, and related issues.
• Build lasting, respectful relationships with families.
